Job Description:
Job Title: Medical Social Worker Profession: Social Work
Specialty: Medical Social Work Duration: Full-Time Shift: Days with
weekends Hours per Shift: 8:00 AM - 4:30 PM Experience: A minimum
of one year related experience in a hospital environment and/or
with transferable skills that apply to a fast-paced and
high-urgency setting. Ability to triage crises and
collaborate/coordinate with collateral agencies. Experience with
protective services, child welfare, long term care, crisis
intervention, chronic illness, hospice care, substance use
disorders, and psychiatric illnesses is a plus. License: Current
Massachusetts Social Work licensure (LCSW/LICSW) is required.
Certifications: A Master’s degree in Social Work (MSW) is required.
Must-Have: Candidates must be local and have not worked for any
facility within the past six months. Description: As a full-time
Medical Social Worker, you will be an integral part of the care
coordination team. You will work in collaboration with the nursing
case management, the clinical team, and internal/external
collaterals. As a member of the interdisciplinary team, you will be
responsible for performing psychosocial assessments. You will
provide supportive counseling and care coordination with respect to
discharge planning and service coordination. This role will involve
working with adult, geriatric, pediatric, and med/surg
populations.
