Summary: Provides individual and group substance abuse treatment relapse prevention and education to substance abuse clients their families and graduated clients at risk of relapse. Works within a residential treatment center and community settings. Collaborates with multi-disciplinary team in assessment diagnosis treatment and discharge planning. Documents clinical activity status and progress. Responsibilities: MAIN DUTIES AND RESPONSIBILITIES: When providing therapeutic services arrives and participates as scheduled: Is alert and responsive to client issued and behaviors. Screens clients to determine appropriateness for program admission. Intakes the client by adhering to Gateway Healthcare Inc. administrative and clinical assessment. Helps clients engage in and respond constructively to treatment. Orients the client and families to the general nature of the program program philosophy services treatment schedule treatment costs to the client rules governing client conduct and program participation confidentiality exceptions client rights and key contacts. Assesses client strengths weaknesses and problem areas for the development of the treatment plan. Prioritizes development of therapeutic alignment. Obtains and Tracks insurance authorization for services. Provides substance abuse and mental health evaluation information including; assessment diagnosis and treatment planning information to supervisor and/or other members of treatment team. Works with Family Court DCYF and Probation to assist youth in achieving expectations of court service plans and community service. Organizes and facilitates treatment team meetings with treatment team client family and additional support people. Develops the treatment plan with the client and treatment team by identifying and ranking problems needing resolution establishing agreed upon short term and long term goals discharge criteria and treatment methods. Participates in treatment planning by: Utilizing assessment information diagnostic results and/or supervisory input in the development of treatment plans. Completing assessment treatment plan and review documents following time and other technical specifications. Modifying treatment planning to meet client needs and changes in situation. Utilizes evidence based and evidence informed treatment whenever applicable. Provides coordinates and monitors treatment services for all assigned cases. Direct individual therapy Direct Family therapy Group Therapy Face-to-face and telephone contact with client. Implementation of care prescribed in the Treatment Plan. Explores problems and their ramifications examines client attitudes and feelings considers alternative solutions decision making and modifies treatment plans to reflect needed changes. Educates by interpreting substance abuse mental health and psychiatric treatment to client and/or clients family thereby reducing fear and other attitudes obstructing acceptance of care and continuation of treatment. Provides psycho-educational services to clients and their families/significant others including provision of information regarding drug and alcohol education cognitive distortion behavior change etc. Provides case management activities which link client agency substance abuse network and community within a planned framework of action toward the achievement of established goals. Identifies the needs of the client that cannot be met by the therapist and assists the client to utilize other Lifespan services as well as support systems and community resources. Refers client and/or clients family to other Lifespan and community resources as needed. Consults with therapists and other professionals in regards to client treatment to assure comprehensive quality care for clients. Develop lead and document psych educational and therapeutic groups as scheduled. Provides relapse prevention education to client and/or clients family. Develops Aftercare Plans with the client prior to discharge in accordance with agency P&P. Assists in adjustment leading to and following discharge. Develops post-discharge Follow-up Plan with the client. Conducts emergency client assessments and refers to inpatient treatment as needed. Coordinates emergency substance abuse and psychiatric services within a timely manner including documenting interventions alternatives considered and response of client/client family and supporting agencies. Facilitates and coordinates Discharge Planning completes documentation according to agency P&P. Adheres to productivity expectations. Case records tasks including assessment treatment planning progress noting and discharge planning are performed by following agency P&P: Documenting with client input treatment implementation and related issues in progress notes and/or treatment plan review documents. Documentation is clear concise and includes all needed information. Documenting all face-to-face contact with the client. Updating of client database reviewing outside correspondence and following client confidentiality and client rights procedures. Assures that each service provided is authorized by contract or payment source prior to the providing the service. Provide direct care support in the milieu as needed to maintain the safety and security of the residents and staff. Maintains familiarity and follows through with care protocols using supervision to clarify any documentation or procedures as needed. Track report and review client outcomes per department or agency protocols. Participate in obtaining collecting and review of client satisfaction data per department or agency protocols. Informs supervisor and any administrative support staff of scheduling conflicts. Attends individual and group supervision as scheduled a minimum of 90%. Completes all mandatory and assigned staff development trainings. Develops individual staff development plan with supervisor. Minimum of 15 hours annually. Staff development plan toward LCDP as appropriate Other duties and responsibilities as assigned or needed. Other information: QUALIFICATION REQUIREMENTS: Masters degree preferred along with Chemical Dependency Licensure. Minimum of Bachelors degree and 3 years experience in the chemical dependency field. CDP LICSW RI Certified Marriage and Family Therapist (CMFT) or RI Certified Mental Health Counselor (CMHC) LCDP required. NECESSARY SPECIAL REQUIREMENTS: Ability to work independently as well as part of a multi-disciplinary team. Self-motivated. Possess an empathetic attitude toward clients and family members balanced by an ability to maintain professional objectivity and commitment to professional values such as self-determination confidentiality and self-awareness. JOB KNOWLEDGE SKILL AND ABILITY: Ability to effectively present information and respond to questions from professionals managers clients family and general public. Ability to read analyze and interpret professional journals policy procedures and governmental regulations. Ability to clearly and cogently document clinical and other information in accordance with Gateway P&P. Ability to write reports and general correspondence. Ability to successfully complete all required training and certifications Oral and written communication skills including speech grammar spelling and legibility are at sufficient level to ensure effective and appropriate communication. Ability to add subtract multiply and divide in all units of measure using whole numbers common fractions and decimals either manually or by use of a calculator. ETHICS: Uses and accesses information in clients health information record or about a client only as necessary to actual duties and then only in accordance with access levels assigned to this position by Gateway Healthcare Inc. from time-to-time and in accordance with Gateway Healthcare Inc. policies rules and procedures as adopted by Gateway Healthcare Inc. from time-to-time. Comply with agency ethics and codes of conduct in all contacts with clients co-workers and the community Observe all agency policies and procedures. INTERPERSONAL FACTORS: Must be able to appreciate the sensitive/private nature and legal status of information about individual clients obtained maintained or used by Gateway Healthcare Inc. and to not access use disclose or request such information about a client unless necessary to the performance of his/her job.
