Requirements: 2 yr of Experience as Case Manager in LTAC/Rehab
ACLS, BLS required • CM experience must be specific for hospital
discharge planning (rehabilitation hospital experience a plus but
not required) • Familiar with EMR and/or documentation expectations
for role (ACMA SOP) • Familiar with Continuity of Care / Transition
of Care requirements (CMS standards )
